MOUNT DORA, Fla. -- The Agency for Persons with Disabilities says the closure of a Lake County facility is imminent.

APD said Thursday that they are selecting a company to take over Carlton Palms.

The Mount Dora facility on Churchill-Smith Lane serves individuals with developmental disabilities and extreme behaviors. 

Nicole Wolff says she took her son, Tristan, out of the facility after APD says Florida Department of Children and Families verified recent findings of abuse and neglect. 

"I can't keep him there any longer. If something would happen to him, I would never forgive myself; I'm done," said Wolff. 

In the complaint filed April 17, APD lists violations that "seriously endangered the health and safety of the residents of the facility" and "the violations have caused the death of one resident." 

"I just want to make sure every parent knows all the details and that the world knows because it shouldn’t happen anymore," said Wolff. 

APD says they're taking action to revoke Carlton Palms's license.

The facility is operated by Bellwether Behavioral Health. We've reached out to them but haven't heard back. 

APD says they will cease all operations in Florida by May 31. 

All residents are being transitioned to other community settings over the next few months.
